If you have been trying for a baby without success, a careful evaluation of both partners is the first step. Many couples need only simple guidance and treatment.

When to see a doctor

  • You have been trying for 12 months without success
  • You are over 35 and have been trying for 6 months
  • Sooner if you have irregular periods, PCOS, very painful periods or previous miscarriages

Evaluation for both partners

Difficulty conceiving can come from either partner, or both, so both are evaluated.

  • For the woman: cycle history, examination, hormone and thyroid tests, ultrasound and ovulation tracking (follicular study), and tests of the fallopian tubes when needed.
  • For the man: a semen analysis, a simple test that gives important information early.

Treatment and guidance

  • Timing intercourse around ovulation
  • Treating conditions such as PCOS or thyroid problems that affect ovulation
  • Treatment to support ovulation where it is suitable
  • Advice on weight, diet, smoking, alcohol and stress for both partners
  • Referral to an advanced fertility centre for IUI or IVF when it is the right next step

Yes. A woman's fertility declines gradually from her early thirties and more quickly after 35, which is why couples over 35 are advised to seek help after 6 months.

What should we bring to the first visit?

Bring any earlier test reports, scans and treatment records for both partners, and note the dates of your last few periods.
